Opportunities abound for pharmacists to participate in comprehensive medication management (CMM) efforts being adopted within value-based care models, demonstrating their value, and achieving cost savings and reimbursements for their practices.

CMM is a patient-centered approach to optimizing medication use and improving patient health outcomes that is delivered by a clinical pharmacist working in collaboration with the patient and other care providers.

Comprehensive medication management ensures that each patient’s medications are individually assessed to determine whether they are appropriate for the patient, effective for the medical condition, and safe, given comorbidities and concomitant medications (two or more meds given at almost or at the same time); and able to be taken by the patient as intended. CMM also incorporates the provision of an individual care plan, working with patients to determine what outcomes to measure, and following up to determine those outcomes.
